Appointment Cancellations
If you cannot keep your appointment time, please call us at least 24 hours in advance so we may give your time to another patient in need of our care. Failure to give 24 hours notice may result in a $25.00 charge.
Your First Visit
On your first visit to our office, we ask you to bring your photo ID (driver's license), medical records, insurance cards and a list of medications you are taking. You will be asked to complete a new patient registration form. This can take 15-20 minutes, so we ask you to arrive prior to your appointment time. This information will assist us in helping you smoothly through your examination.

New patients can download the patient forms needed for a first appointment using the links below. Forms can thus be printed, completed, and brought to the first appointment.

Payment
We accept many insurance plans, however, patients with HMOs and PPOs must inquire regarding our acceptance of their specific plan. In addition, those with Meidcaid must inquire if their specific plan is accepted as well. Our helpful staff will tell you if we participate in your particular health plan. We accept cash, check, VISA, MasterCard, American Express and Discover. Our policy is that you pay any amount not covered by your insurance at the time of your visit. Our billing department will determine this amount at the end of your visit.
Telephone Calls
When calling our office, it is important that you give our receptionist your name (or the patient's name) and the nature of the problem. Our receptionist will then direct your call to the person best able to answer your questions. It may not always be possible to talk to your physician when you call. If this occurs, our receptionist will bring your questions to the attention of your physician or the nurse as soon as possible.
Please understand it is impossible to properly diagnose and treat your condition without an examination. In addition, it is our policy not to release test results over the phone.
Prescriptions
When calling to renew your prescription, please call at least 3 days before you are out of your medication. Please have the name and dose of your medication as well as the name and phone number of your pharmacy available for our staff. Please restrict your calls to Monday through Friday, 9am to 3pm.
Form or Copy Fee
There is a fee to complete forms and paperwork (such as disability and other employment forms). There will also be a charge to copy medical records and documentation.

If you require hospitalization, the doctor you see in our office can help arrange your admission whenever possible. During your hospital stay, you will be seen daily by either the admitting physician or one of our associate physicians to assure your comfort and recovery. If you are hospitalized for other reasons and have the need to see you’re A.I.N. neurologist, this can be arranged by notifying your doctor or calling our office.
